Intake form
You can contact us by phone or complete this form to schedule an evaluation with one of our therapists. This form will provide us with your contact information and allow you to share your current concerns and questions, as well as the goals you hope to achieve in the therapeutic process.
Contact from therapist
You will receive a call within 24 hours from one of our therapists to review the intake form and schedule an initial session.
Evaluation
The first session will be between 1 ½ and 2 hours long. You will accompany your child throughout the evaluation (as you are invited to do in all sessions). Your child will participate in a standardized evaluation – typically involving table-top and gross motor activities – and a play-based evaluation. The play-based evaluation will take place in the sensory gym and/or with toys.
Parent meeting
The therapist will schedule a meeting with you within one week of the evaluation. During this meeting, you will hear the therapist’s analysis of the evaluation, review program options, and set functional goals.
Start therapy sessions
Once program options are chosen and goals are set, you and your child will start sessions with your therapist!
